Effectiveness of case management and post-acute services in older people after hospital discharge

Patients aged over 65 who were discharged from the hospital and who required community services after discharge were studied to determine if coordination of those services via the Post-Acute Care program improved patient outcomes in Victoria, Australia. Although intervention did not affect mortality in the six months following discharge, patients in the coordinated services group showed much better quality of life scores than those who received only traditional discharge planning.
