Adölasan idiopatik skolyozun cerrahi tedavisinde anterior ve posterior enstrümentasyon sonuçlarının karşılaştırılması

ÖZET Adölesan İdiopatik Skolyozun Cerrahi Tedavisinde Anterior ve Posterior Enstrümantasyon Sonuçlarraın Karşılaştırılması Omurganın en yaygın deformitelerinden biri olan skolyoz eski çağlardan beri bilinmekte olup hastalar üzerinde sosyal ve psikolojik baskı oluşturmaya devam etmektedir. İdiopatik skolyoz kompleks üç boyutlu bir deformitedir. Doğal seyri konusunda yeterli bilgi sahibi olunmakla birlikte, etiyolojisi tam anlamıyla ortaya konamamıştır. Tüm skolyozların %80'ni oluşturan AIS' de bel ve sırt ağrısı, dengenin bozulması ve kozmetik deformite doktora başvuru sebebidir. Vertebral kolonun hareketliliğini koruyarak eğriliği düzeltmek her zaman mümkün olmamaktadır. Cerrahi girişimler hareketli segmentleri ortadan kaldırmakta, hatta bazen belirgin denge bozukluklarına neden olabilmektedir. Bu nedenle AIS tedavisinde arayışlar halen devam etmektedir. Bu çalışmanın amacı cerrahi tedavi gerektiren AIS hastalarında üçüncü kuşak enstrümantasyon sistemi kullanılarak gerçekleştirilmiş olan anterior ve posterior girişimleri retrospektif olarak değerlendirerek, deformitenin her üç düzlemde düzeltilmesinde etkinlikleri, elde edilen düzelmenin takip süresi içindeki seyri, her iki yöntemin denge parametrelerine etkilerini, ortaya çıkmış olan komplikasyonlan belirlemek, her iki yöntemin birbirilerine göre avantaj ve dezavantajlarını ortaya koymaktır. Her iki girişimde de uygun alt ve üst enstrümantasyon seviyeleri seçildiğinde koronal planda ve denge parametrelerinde tatminkar düzelme sağlanmakta, fizyolojik sagital eğimler korunmaktadır. Koronal ve transfers düzlemde, denge parametrelerinde daha anlamlı düzelme sağlaması, daha az segmentin fuzyona uğratılması dekompansasyon riskinin daha düşük olması, nörolojik hasar riskinin çok az olması, zayıf hastalarda cilt altında implant belirgenleşmesi riskinin olmaması anterior girişimin avantajlarıdır. VI ABSTRACT Comparison of Anterior and Posterior instrumentation in the Surgical Treatment of Adolescent idiopathic Scoliosis Scoliosis has been known as one of the common spinal deformities that has been a social and psychological problem since old ages. Idiopathic scoliosis is a complex three dimentional deformity. Although there is enough information about the progression, the ethiology of the disease has not still been clarified. Back pain, imbalance, cosmetic deformity are the leading complaints of AIS which is 80 % of all the scoliotic cases. Correction of the deformity while preserving the mobility of the spine is not always possible. Surgical procedures often interefering with the mobile segments sometimes cause evident imbalance. Therefore treatment of AIS is still an issue of interest. The aim of this study is to evaluate the anterior and posterior interventions using third generation instrumentation system on the patients who required surgical treatment, the effect on correction of deformities on each three planes, the follow up of the provided correction, the effect of both techniques on balance parameters, to determine complications, to expose the advantages and disadvantages of both techniques retrospectively. Both procedures provide satisfactory balance parameters and correction in coronal plane, and the phsiologic sagittal curves are preserved if the proximal and distal levels of instrumentation are appropriate. Significant improvement of balance paraneters on coronal and transverse planes, more limited fusion of segments, having a lower decompensation and neurologic deficit risk and giving no chance of feeling the instrument under the skin of even thin patients are the advantages of anterior instrumentation.
