Afrontamiento de la muerte de un niño: los cuidados pediátricos paliativos desde la Atención Primaria

Los cuidados paliativos pediátricos (CPP) son aquellas actuaciones destinadas a niños y adolescentes que padecen una enfermedad incurable, progresiva e incapacitante, y que se inician en el momento del diagnóstico y se mantienen durante toda la evolución de la enfermedad. Estas actividades son realizadas por un equipo multidisciplinar de profesionales que tratan simultáneamente al niño enfermo y a su familia, desde un punto de vista integral, abarcando todas sus necesidades físicas y psicológicas, personales o comunitarias. En su inicio han estado muy ligadas a la labor hospitalaria e incluso la atención domiciliaria se realiza desde el ámbito del hospital. La Atención Primaria, como atención del niño y su familia en su entorno biopsicosocial, se perfila como un colaborador excelente dentro del equipo multidisciplinar, ya que recoge los conocimientos del niño y la familia previos al diagnóstico de la enfermedad, el seguimiento, el conocimiento del entorno escolar y social, y el trabajo con los hermanos y familia después del desenlace de la misma. Analizamos la actuación de la Atención Primaria dentro del equipo de Cuidados Paliativos Pediátricos en el afrontamiento del proceso de enfermedad terminal y muerte de un niño, con consideraciones sobre las actitudes, la comunicación, el proceso de duelo y el acompañamiento del niño y la familia en todas las fases de la enfermedad y después de esta. Pediatric palliative care (PPC) are actions aimed at children and adolescents who suffer from an incurable, progressive and disabling disease, and start at the time of diagnosis and are maintained throughout all the evolution of the disease. These activities are conducted by a multidisciplinary team of professionals treating simultaneously the sick child and his/her family, in a comprehensive way, covering all their needs, physical and psychological, with a personal or community perspective. In its beginning, this attention has been hospital centered including home visits. Primary care, such as the care of the child and his/her family in their bio-psycho-social environment, is emerging as an excellent collaborator within the multidisciplinary team, it collects the knowledge of the child and the family prior to the diagnosis of the disease, the monitoring, the knowledge of the school and social environment, and works with the siblings and family after the outcome. We analyze the performance of primary care within the pediatric palliative care team, in coping with the process of terminal illness and death of a child, with considerations about attitudes, communication, the grief process and the accompaniment of the child and family in all phases of the disease, and after it.
