Weichteildefekt als Komplikation nach Knieendoprotheseninfektion

Schwer eradizierbare Infektionen sind nach Knieprothesen-Implantation eine bekannte Komplikation. Ziel dieser retrospektiven Arbeit war es, einen Therapiealgorithmus zu erarbeiten. Das Patientenkollektiv wurde in Gruppe A (orthopädisch-chirurgische Therapie und plastische Deckung) und Gruppe B (rein orthopädisch-chirurgische Therapie) unterteilt. Es konnte festgestellt werden, dass Patienten mit einem Knieprotheseninfekt - sowohl Früh- als auch Spätinfekt - ab einem Weichteildefektstadium 2 (nach Laing) durch die Kombination aus plastisch-chirurgischer Deckung und Komponentenwechsel oder -ausbau (KOA) optimal therapiert waren und die niedrigste Komplikationsrate im Verlauf hatten. Bei Patienten mit einem Protheseninfekt ab einem Weichteildefektstadium 2 (nach Laing) empfehlen wir daher die Durchführung eines KOA zusätzlich zur plastisch-chirurgischen Weichteildeckung. Deep infection with soft tissue defect are a well known problem after total knee arthroplasty. The aim of this retrospective study was to develop a therapy algorithm. The patient collective was devided into group A (orthopaedic-surgical therapy as well as coverage of the soft tissue defect) and group B (orthopaedic-surgical therapy). The analysis showed that patients with periprosthetic joint infection from a soft tissue defect grade II (according to Laing) - early infection as well as late infection - were optimally treated with soft tissue reconstruction combined with prosthesis component removal or change (KOA) had the lowest complication rate. Therefore soft tissue reconstruction combined with prosthesis component removal or change (KOA) in patients with periprosthetic joint infection from a soft tissue defect grade II (according to Laing) is recommended.
