Degloving injury of the mandible

A 13-year-old girl presented at an emergency department after she had fallen on her face when she fell from her horse. During physical examination no apparent extraoral injury was visible, such as lacerations, bruises or oedema. However, intraoral examination revealed extensive laceration of soft tissue. The diagnosis was a degloving injury of the mandible, which is very rare. The treatment consisted of debridement, suturing of the mentalis muscle, the approximate closing of the mucosa, accompanied by treatment with broad-spectrum antibiotics. The injury healed well without any residual complaints. This case underlines the importance of intraoral examination during the evaluation of patients with potential facial injury.Een 13-jarig meisje presenteerde zich op een spoedeisende hulppost nadat ze door een val van een paard op haar gezicht was gevallen. Bij extraoraal onderzoek werden geen afwijkingen gezien zoals laceraties, blauwe plekken of oedeem. Bij intraoraal onderzoek werd echter een uitgebreide laceratie gezien van de weke delen. De diagnose was degloving-letsel of ‘afstropings­letsel’ van de mandibula, hetgeen erg zeldzaam is. De behandeling bestond uit uitgebreid débridement, het hechten van de musculus mentalis, het approximerend sluiten van de mucosa met daarnaast behandeling met een breedspectrumantibioticum. Het letsel is restloos genezen. Deze casus onderstreept het belang van intra­oraal onderzoek tijdens de evaluatie van een patiënt met mogelijk aangezichtsletsel.
