Cyanotic Raynaud's phenomenon with conventional but not with liposomal amphotericin B: three case reports

Abstract
- Summary. Despite its common adverse effects intravenous (i.v.) amphotericin B is an indispensable antifungal drug in childhood oncology. We report here on three cases of painful cyanotic Raynaud's phenomenon after i.v. administration or inhalation of amphotericin B. A liposomal i.v. preparation of amphotericin B was well tolerated by the infants. Spasms of peripheral vessels mediated by thromboxane A2 could be responsible for the Raynaud's phenomenon. Hence, inhibitors of prostaglandin synthesis are suggested for therapy. Zusammenfassung. Trotz seiner bekannten Nebenwirkungen bei intravenoser Applikation ist Amphotericin B ein unverzichtbares Therapieelement in der Behandlung schwerer Mykosen in der Kinderonkologie. Wir berichten uber drei Falle von schmerzhaftem, zynotischem Raynaud-Phanomen in zeitlichem Zusammenhang mit intravenoser und inhalativer Verabreichung von konventionellem Amphotericin B. Die intravenose Gabe von liposomalem Amphotericin B wurde ohne Nebenwirkungen toleriert. Thromboxan A2-vermittelte Gefasspasmen durften verantwortlich sein fur das Raynaud-Phanomen. Eine logische Therapieoption ware demzufolge die Verabreichung von Inhibitoren der Prosta-glandinsynthese.
