Pelvic hematoma masquerading as rectal abscess: report of a case

Abstract

Postpartum pelvic hematomas, collections of blood anywhere in the pelvis between the pelvic brim and the perineum, are much more common than is evident from reports in the literature. They vary in significance from minor local vulvar and perineal hematomas that are readily visible to massive hidden retroperitoneal hematomas that may be fatal. Pain is a frequent symptom. With the vulvovaginal or vaginal hematoma, the chief complaint may be rectal pain. The proctologist should be cognizant of the entity and the similarity of its presenting symptoms to those of rectal abscess. A case of a patient who had postpartum pelvic hematoma is described. The initial symptom was severe rectal pain. The hematoma was drained by an unusual route, with a very satisfactory result.
