Abstract
- Antecedentes La técnica de reemplazo de la raíz aórtica con preservación valvular ha ganado popularidad en los últimos años, debido a su seguridad y ausencia de requerimiento de anticoagulación. El objetivo de este estudio fue determinar la morbilidad y mortalidad tempranas y tardías en pacientes sometidos a reemplazo de la raíz aórtica con preservación valvular. Metodología Estudio descriptivo de cohorte retrospectiva, de 118 pacientes sometidos a cirugía de Tirone David en el periodo enero 2002 a febrero 2020. El seguimiento se realizó mediante valoración ambulatoria y consulta en bases web gubernamentales para determinar el tiempo libre de reintervención y recurrencia de la insuficiencia valvular, así como la sobrevida, los cuales se analizaron mediante el método de Kaplan-Meier. Resultados La mediana de edad fue de 51 años, 28 tenían Síndrome de Marfan, la clase funcional de NYHA > II fue del 85%, insuficiencia aortica >2+ en el 77% de los pacientes. El diámetro promedio de la raíz aórtica fue de 56 mm +/- 9.6, la mortalidad operatoria fue de 0.8%. La sobrevida a 14 años fue de 84% (IC 95% 67-93). El seguimiento ecocardiográfico se completó en 78% de los casos y reveló una incidencia de insuficiencia aórtica postoperatoria de 3.2%. La libertad de insuficiencia aórtica a 120 meses fue de 98% (IC 95% 86-99). Conclusiones El reemplazo de la raíz aórtica con preservación de la válvula aórtica es seguro en esta cohorte, con una tasa de mortalidad quirúrgica inferior al 1% y libertad de mortalidad a largo plazo de 78%, similar a las reportadas en la literatura. Background Aortic valve-sparing root replacement (AVSRR) has gained popularity in recent years, mostly owing to its proven safety and lack of need for anticoagulation. The aim of this study was to determine the early and late morbidity and mortality associated with AVSRR in a single institution in Latin America. Methods This is a descriptive study with a retrospective cohort of 118 patients who were submitted to Tirone David procedure between January 2002 and February 2020. Patient follow-up was done in outpatient clinic, through telephone interviews and government databases to define the free time of reoperation, valve regurgitation recurrence and event-free survival, which were analyzed using Kaplan-Meier. Results The median of age was 51 years old, 28 patients had Marfan’s syndrome, the NYHA class was ≥ II in 85%, the degree of aortic regurgitation was >2+ in 77%. The mean aortic root diameter was 56mm ± 9.6, operative mortality was 0.8%, survival at 14 years was 84% IC 95% (67-93). Echocardiographic follow-up was 78% completed and revealed a postoperative AR (aortic regurgitation) in 3.2%. Freedom from AR at 120 months was 98% IC 95% (86-99). Conclusions The aortic valve-sparing root replacement (AVSRR) is safe in this cohort, with a surgical mortality rate less than 1% and a long-term mortality freedom of 78%, similar to the evidence reported in the literature. 2021-04-19 13:45:02: Script de automatizacion de embargos.
