Abstract

A 38-year-old man with a history of cocaine abuse had an evaluation for chest pain. Myocardial infarction was ruled out, and he underwent an exercise thallium myocardial perfusion SPECT study that suggested myocardial ischemia in the left anterior descending (LAD) artery territory. Cardiac catheterization showed a minimal coronary disease but a coronary artery fistula communicating the LAD artery with the main pulmonary artery. Toxic screen for cocaine was negative.
