Effects of Chai-Qin-Cheng-Qi decoction on regulating protein kinase C pathway in colon smooth muscle cells of rats with acute necrotizing pancreatitis of rats

Abstract

s / Pancreatology 16 (2016) S1eS63 S40 Background: Prediction of the malignancy of intraductal papillary mucinous neoplasm (IPMN) remains difficult, especially for themixed type. Objective: The aim of this study was to evaluate the usefulness of EUS to predict malignancy for resection of branch duct-type (BD)-IPMN and to compare them with spiral CT and MRCP. Methods: A prospectively collected database of patients with resected BD-IPMN who receive EUS, CT and MRCP before the operation was analyzed to compare factors as predictors of malignancy. Results: Of 42 patients with BD-IPMN, 14 were diagnosed as malignancy according to surgical pathology. The sensitivity (0.72) and specificity (0.75) of EUS were superior to those of CT (0.62 and 0.72) and MRCP (0.52 and 0.68). Multiple logistic regression analysis showed that main pancreatic duct dilatation greater than 5mm (HR 5.56, 95% CI 2.45 ~ 9.62; P < 0.001), mural nodules (HR 4.28, 2.15 to 10.21; P < 0.001) and carbohydrate antigen 19-9 level above 37 units/ml (HR 3.02, 1.25 to 10.90; P 1⁄4 0.001) were independent predictors of BD-IPMN malignancy. Conclusion: EUS has better performance to classify BD-IPMN than CT andMRCP. Clinical significance of the variable factors associatedwith a risk of malignancy indicates the need for a tailored approach in the management of patients with BD-IPMN.
