Integrated pharmaceutical care in the community: (how) can we turn vision into reality?

This article provides a summary of main messages of the annual Steve Hudson memorial lecture presented by Tobias Dreischulte at the 42nd European Society of Clinical Pharmacy symposium in Prague in 2013. An exploration of what is meant by integrated pharmaceutical care, the design and pilot testing of a model for continuous quality improvement of medication use designed to facilitate care integration, and opportunities and challenges of implementing the model in practice are discussed.
