Abstract 9420: A Striking Contrast Between Lipid Therapy Prescribing Patterns versus Achieving Optimal Lipid Levels in Black and Hispanic Populations Following Acute Coronary Events

Abstract

Introduction: Guideline directed lipid therapy (GDLT) for patients following acute coronary syndrome (ACS) is well-established secondary prevention. Historically disparities in secondary prevention have been observed among minority populations, however there is limited understanding of GDLT and resultant impact on achieving optimal LDL levels. Hypothesis: GDLT may not proportionally result in achievement of LDL Methods: A cross-sectional study was employed across all University of Pennsylvania Health System from 2018 to 2019. Electronic medical records were queried for all admitted patients diagnosed with ACS. Data was collected for age, gender, race, GDLT, and lipid profiles up to 1-year post-discharge. Results: We identified 3386 patients with whom 2264 (66.8%) were male and 1122 (33.1%) were female. The majority was white representing 63.5% (2152), followed by 21.2% (718) Black, Conclusions: Black and Hispanic groups were prescribed GDLT at a greater rate than the White population. Despite this, less than 50% of Black and Hispanic groups achieved optimal LDL levels, about 10% less than the White population. Furthermore, non-statin therapy was prescribed slightly less than the White population. We observed disparities among racial groups in achieving optimal LDL levels despite being prescribed GDLT at a higher rate. Future investigation and a holistic approach to existing racial disparities will allow for improved secondary prevention.
