Abstract

Introduction Sarcopenia, evaluated by body composition analysis through CT scan at the third lumbar vertebra level (L3–CT), is a promising prognostic stratification parameter in patients undergoing Transcatheter Aortic Valve Implantation (TAVI). Conversely, only a few data are available on the impact of visceral fat, a metabolic active tissue associated with unfavorable cardiovascular outcome. AIMS. We investigated the correlation between baseline visceral fat and 30–days mortality in patients undergoing TAVI. Methods We prospectively collected anamnestic and clinical data of 81 patients (42.5% males) who underwent TAVI in 2010–2020. Trough retrospective analysis of 55 available CT–L3 images (Fig 1), we measured visceral adipose tissue index (VATi), expressed as cm2/m2. The sample was classified according to the median VATi value (>50percentile VS Results VATi median value was 57.4 cm2/m2. Patients with VATi>50p were younger (83.3 [76.5–87] vs. 86 [83–88] years, p=0.020) and showed more often pre–operative NYHA class >2 (78% vs. 40%, p=0.021) as compared with patients with VATi50p group presented a higher 1–year mortality (21.4% vs. 3.7%, HR=7.01, p=0.05), whereas no differences were found in 30–days mortality (3.6% vs. 3.7%, p=0.745). Conclusions Visceral fat is associated with a higher 1–year all cause mortality in patients treated by TAVI. Further studies on wider populations are needed to confirm this finding. Adding the evaluation of visceral fat to the clinical practice could help to optimize pre–operative mid–term mortality risk stratification in this setting.
