La comunicació com a eina terapèutica en les cures pal·liatives: revisió sistematitzada

Introducció: la situació de final de vida és un fet inevitable i, les cures proporcionades a aquests pacients varien dels uns als altres. Les cures pal·liatives intenten aportar una major qualitat de vida als pacients durant el temps de vida restant, i per tal que això sigui possible, s’utilitzen diverses estratègies, entre les quals destaca la comunicació com a eina terapèutica. Objectiu: descriure els beneficis i com influeix la comunicació entre l'infermer/a, el pacient i la família en situació de final de vida en una malaltia terminal. Material i mètodes: s’ha realitzat una recerca bibliogràfica en les bases de dades PubMed, Scielo, Cuiden i Scopus, prioritzant informació publicada en els darrers deu anys. El risc de biaix o la qualitat dels articles s’ha mesurat a través de CASPE. S’ha seleccionat aquella que parlés sobre la comunicació com a eina terapèutica en cures pal·liatives, així com les habilitats necessàries perquè aquesta sigui efectiva i les possibles barreres existents en tot aquest procés comunicatiu i terapèutic. Resultats: la comunicació es considera una eina de cuidatge molt important encara que invisible. Fomenta l’establiment d’un vincle terapèutic i d’ajuda des d’una perspectiva humana, factor imprescindible per comprendre al malalt i el seu entorn, les seves pors i les seves preocupacions així com facilitar-li les eines essencials, per afrontar la seva situació actual i futura davant d’un mal pronòstic, respectant sempre la seva autonomia i la seva dignitat com a persona. Conclusió: s’ha pogut demostrar que la comunicació com a eina terapèutica aporta múltiples beneficis, no tan sols pel pacient amb malaltia terminal sinó també al seu entorn més proper. A més, es destaca la importància de disposar d’habilitats humanes i comunicatives per afavorir el vincle i evitar les possibles barreres comunicatives conegudes. Introduction: the end-of-life situation is an inevitable fact and, the care provided to these patients varies from one to another. Palliative care tries to provide patients with a better quality of life for the rest of their lives, and various strategies are used to make this possible, including communication. Objective: describe the benefits and how the communication between the nurse, the patient and the family in a situation of end of life with a terminal illness influences. Methodology: a bibliographic search has been carried out in the PubMed, Scielo, Cuiden and Scopus databases, prioritizing information published in the last ten years. Item quality has been measured through CASPE. The one that talks about communication as a therapeutic tool in palliative care has been selected, as well as the skills needed for it to be effective and the possible barriers that may exist throughout this communicative and therapeutic process. Results: communication is considered a very important tool of care, although often invisible. It encourages the establishment of a therapeutic and supportive link from a human perspective, an essential factor in understanding the patient and his environment, his fears and concerns as well as providing him with the essential tools to deal with their current and future situation in the face of a bad prognosis, always respecting their autonomy and their dignity as a person. Conclusion: communication as a healing tool has been evidenced to provide multiple benefits, not exclusively for the terminally ill patient but also for their immediate environment. In addition, the importance of having human and communicative skills to foster the bond and avoid possible existing communication barriers is emphasized.
