•Treo-based conditioning shows better overall survival versus busulfan in MDS patients.•Treosulfan recipients had better relapse free survival and event free survival.•Study reveals treosulfan's efficacy and safety in stem cell transplantation.•First study to evaluate treosulfan with ATG and PTCY, supporting its growing evidence.
