Integration of a Pharmacist Into a Case Management Program for Medically Fragile Children.

The article focuses on the Community Case Management (CCM), a case management model for medically fragile children formulated by the Massachusetts Office of Medicaid (MassHealth). It explores the role of nurse case managers who visit the home of a medically fragile child. It also provides a brief overview of the condition and complication of a medically fragile child. A pharmacist was added to the CCM team in September 2006 to provide operational and clinical services.
