CONSERVATIVE SURGICAL APPROACH AND RECURRENCE OF GLANDULAR ODONTOGENIC CYST - CASE REPORT.

Abstract
- The glandular odontogenic cyst (GOC) is a rare cystic condition that can exhibit aggressive behavior. A 46-year-old man with a brown complexion presented with an increased volume on the right side of his mandible. Cone-beam tomography revealed a hypodense, multilocular image with well-defined margins, affecting the symphysis and mandibular body, and involving the roots of dental elements from 32 through 46. An incisional biopsy confirmed the diagnosis of GOC. The treatment involved the removal of the lesion through curettage and peripheral osteotomy, with a focus on preserving the affected teeth. However, after 6 years, the lesion recurred, prompting a conservative but similar surgical procedure. Subsequently, after 6 months, a lesion was observed in the lingual region of teeth 44 and 45, leading to another enucleation with peripheral osteotomy in that area. To monitor the patient's condition and address the possibility of recurrence, clinical and imaging follow-up examinations are conducted biannually, as approximately 55% of cases show recurrence following conservative treatment.
