An evidence-based health workforce model for primary and community care.

The article presents information on the development of an evidence informed, needs-based health workforce model. The model has been developed for supporting best-practice interdisciplinary chronic disease management in both the primary and community care setting using diabetes. The model reflects the diversity in the community as well as clinic populations and also parsimonious and clear to present and operationalize.
