Abstract

The article discusses the limitations of using major adverse cardiovascular events (MACE) as the primary outcome measure in cardiovascular trials. It highlights the challenges of including multiple events of varying clinical relevance in a composite endpoint like MACE, which may mask the effects of more clinically relevant outcomes. The authors propose an alternative approach that includes death and quality of life in a hierarchical composite outcome to better reflect what patients desire from healthcare delivery. The article emphasizes the importance of assessing quality of life as a primary endpoint in cardiovascular trials to overcome the limitations of traditional MACE outcomes.
