Abstract

This article explores the implementation of social prescribing in a Canadian regional health system, specifically in the province of British Columbia. The social prescribing model aims to support healthy aging by integrating community services with traditional acute care. Fraser Health, a regional health authority, has established a team dedicated to integrating social prescribing into the health system through collaboration with various partners. The article emphasizes the importance of funding stability, partnerships, organizational capacity, program evaluation, and strategic planning in sustaining and expanding the social prescribing model.
