Abstract

The University of Toronto has released updated guidelines for the management of Helicobacter pylori infection in children and adolescents. The guidelines recommend invasive testing with strain antimicrobial susceptibility analysis for diagnosis and selection of eradication therapy. Molecular methods are acceptable for detecting infection and antibiotic resistance. Noninvasive tests can be used as a screening method for children with a family history of gastric cancer. Testing for H. pylori is no longer recommended when investigating chronic immune thrombocytopenic purpura, inflammatory bowel disease, celiac disease, or eosinophilic esophagitis. Treatment should be based on antibiotic susceptibility testing, and regimens containing clarithromycin should be avoided if susceptibility testing is unavailable. The guidelines aim to address the decreasing prevalence of infection, increasing antibiotic resistance, and emerging evidence regarding complications of H. pylori infection in children.
