Abstract

A recent study conducted at Robert-Debre University Hospital in Paris, France, examined the planned mode of delivery for twin pregnancies and the management of second twin delivery. The study included 8,823 women with twin deliveries and found that for twin pregnancies with a cephalic first twin at 32 weeks of gestation and beyond, severe neonatal morbidity was higher in the planned cesarean delivery group compared to the planned vaginal delivery group. However, planned vaginal delivery was not associated with increased severe neonatal morbidity in cases of breech presenting first twin after 32 weeks of gestation or with decreased survival without severe neonatal morbidity in cases of delivery before 32 weeks of gestation. The study concluded that planned vaginal delivery is the recommended mode of delivery for the majority of pregnant women with twins, regardless of the first twin's presentation and gestational age at delivery. The choice of management for the second twin will depend on the obstetrician's preferences. [Extracted from the article]
