An Old Concept With New Challenges: An Innovative Approach to Paper Documentation.

The article focuses on electronic health record (EHR), and discusses a study conducted on the importance of paper documentation for new nurses. In the study, a patient was presented to the emergency department with shortness of breath, and the nursing students were told that the EHR was not available. The faculty told the students that all care provided to the patient would be documented using paper. The study found that handwritten orders improved patient safety.
