Abstract

Commentary Gastroptosis, defined as downward displacement of the stomach into the pelvis, is a rare anatomic abnormality often presenting with vague abdominal symptoms similar to gastric motility dysfunction. The diagnosis can be difficult when standard modalities are used, such as bloodwork, endoscopy, and abdominal US. However, barium imaging studies often provide a striking diagnosis. In this case, a patient with abdominal pain underwent an extensive work-up with endoscopy, EUS, and manometry before a barium study confirmed the rare diagnosis of gastroptosis with abnormal gastric emptying on gastric emptying scintigraphy. Vague abdominal symptoms, especially when related to rare conditions, can often require a multimodality work-up for successful diagnosis. However, it is important to remember that despite modern medicine’s diagnostic advances, clues to a patient’s pathologic condition can still often be found in the history; in this patient, having her symptoms relieved by abdominal contractions and standing upright were clues to the underlying rare diagnosis of gastroptosis.
