Abstract

Idiopathic pneumonia syndrome (IPS) is a serious complication after hematopoietic stem cell transplantation. Despite the high mortality rate with medical management, there have been no reported cases of lung transplants for patients with IPS. We report a case involving a 44-year-old woman who developed IPS 5 months after hematopoietic stem cell transplantation for myelodysplastic syndrome. Despite aggressive medical management, the patient required intubation and was administered extracorporeal membrane oxygenation while awaiting recovery. However, her condition continued to deteriorate, and she subsequently underwent a double lung transplant with uneventful recovery. With the high mortality of medically managed IPS, lung transplant could prove to be lifesaving.<br /> (Copyright © 2020 The Society of Thoracic Surgeons. Published by Elsevier Inc.
