Abstract

The article discusses the data and conclusions of Herbert William Heinrich, who was a pioneer in bringing attention to workplace safety. Heinrich became popular for his 1931 book "Industrial Accident Prevention: A Scientific Approach," in which he stated that 88 percent of accidents are caused by unsafe acts of persons. It notes that Heinrich's domino theory presents five factors for an accident to occur which include fault of the person, mechanical or physical hazard, and the accident.
