Patient-Reported Care Coordination: Associations With Primary Care Continuity and Specialty Care Use.

The article focuses on a cross-sectional study of Medicare enrollees suffering from chronic diseases in an integrated health care delivery system in Washington. It informs that the study emphasized on the association between care coordination and continuity of primary care.
