Abstract
- Ischaemic heart disease and heart rhythm disturbances are common in patients with chronic renal insufficiency. Renal replacement therapy is the specific field where both myocardial ischaemia and arrhythmias may occur, enhance and influence each other. Regarding that, authors studied 42 patients who had chronic renal insufficiency, on maintenance haemodialysis. Patients have been assessed by extradialytic rest electrocardiography, intradialytic Holter EKG examination and transthoracic echocardiography before dialysis. The goals of the research were recording background ischaemia versus intradialytic ischaemic variability, intradialytic heart rhythm disturbances, as well as impaired left and right ventricle function associated with arrhythmias. Authors found a low prevalence (16,7%) of intradialytic ischaemic variability vs background ischaemia (45,2%), suggesting that haemodialysis does not lead to myocardial ischaemia and does not increas previous ischaemia. On the other hand, variable ischaemia was associated to heart rhythm disturbances in all patients, especially premature heart beats (100%). Dialysis-related heart rhythm disturbances had a high-rate frequency (97,6%), in correlation with the presence of impaired myocardial function and not with the sort of it. [ABSTRACT FROM AUTHOR]
