Abstract

Purpose: To assess anatomic and functional results after surgical management of retinal detachments from inferiorly located breaks. Materials and Methods: Twenty eyes of 19 patients (8 female, 11 male) with retinal detachment due to inferiorly located retinal breaks were included in this study. Eight of the eyes without significant proliferative vitreoretinopathy (PVR) underwent conventional surgery with encircling scleral band and local buckle, while another 8 eyes with grade C1 or worse PVR underwent pars plana vitrectomy (PPV) and intravitreal tamponade in addition to encircling scleral buckle. Four eyes that had been operated on in other centers but showed recurrence were also re-operated using PPV in our clinic. The results of our surgical intervention were evaluated in terms of anatomy and functionality. Results: Conventional retinal surgery resulted in anatomic success in 6 out of 8 eyes with a single operation. Another 2 eyes were further operated on to attach the retina. Anatomic success was achieved in 7 of 8 eyes that underwent a combined operation in the first attempt, and one other in the second attempt. In the eyes that were referred to our clinic for re-operation, anatomic success was obtained in 2 of the 4 eyes with one operation, and in other one with a further operation. No success was achieved in the fourth eye. Overall, retinas were found to be attached in 19 eyes (95%), while visual acuity improved in 14 eyes (70%) and remained the same in 6 eyes (30%) at the final examination. Conclusion: In eyes with retinal detachments with inferiorly located retinal tears, conventional surgery seems to suffice in eyes without significant PVR, while scleral buckling combined with PPV is appropriate for anatomic and surgical success in eyes with significant PVR.
