Intracranial Candidal Granuloma in an Immunocompetant Female.

Abstract

We present a rare case of intracranial candidal granuloma in an immunocompetant female. 24 year old immunocompetant female presented to the neurology opd with signs and symptoms of left cavernous sinus syndrome. On investigation blood counts, blood sugar liver and renal function tests, chest X-ray was normal; hiv was negative. CT brain contrast showed irregular hyperdense lesion arond cavernous sinus with normal paranasal sinuses, there was no history of any local nasal ayurvedic treatment or treatment with antibiotics or steroids prior to the present illness. Patient was under gone lesionectomy and histopathology was suggestive of candidal granuloma. Patient was started on amphotericin, unfortunately patient not tolerated amphotericine and later expired. Lessons learned from our case are: 1. Clinical diagnosis of fungal infections in immunocompetant is extremly difficult; biopsy and histopatholgy is gold standard. 2. Invasive fungal infections can occur even in immunocompetant people because of tropical climate that was reported in previous studies. 3. Treatment is difficult as most of the conventional antifungale have very poor side effect profile. Emerging antifungals with more efficacy and less side effects like capsofungin and voriconazole will also be discussed. [ABSTRACT FROM AUTHOR]
