Abstract

Diabetic patients carry a large array of comorbidities that negatively impact their quality of life. Among these, it has been highlighted that diabetic individuals are highly prone to developing pathologic fractures. Evidence suggests that individuals with type 1 diabetes mellitus (DM1) have a greater risk in this regard than those with type 2 diabetes mellitus, which may be due to the absolute insulin deficit observed in the first group, which has implications on bone health, especially at an early age. Nevertheless, it should be noted that multiple mechanisms are involved in the reduction of bone health in diabetic individuals. Beyond the increased risk of fractures, diabetic patients also tend to have less satisfactory and more prolonged recoveries. Given this panorama, the best approach would be a preventive focus that would allow individuals at high risk in order to offer the necessary preventive measures, but further research on paraclinical markers with better predictive potential is necessary to stratify the populations by high and low risk. The objective of this review is to evaluate the pathophysiologic mechanisms explaining the development of these complications in the context of patients with DM1; likewise, the current evidence will be revised correlating DM1 with complications such as fractures.
