Abstract
- Objective To explore the treatment effects and safety of transthoracic ultrasound-guided closure combined with percutaneous coronary intervention (PCI) in patients with coronary artery disease and patent foramen ovale (PFO). Methods A retrospective analysis was conducted on the clinical data of 110 patients who underwent PFO closure combined with PCI from February 2021 to March 2024. Among them, 50 patients underwent X-ray-guided PFO closure (X-ray group), and 60 patients underwent transthoracic echocardiography (TTE) -guided PFO closure (TTE group). The study compared perioperative cardiac function indices, shunt status, complication rates, and adverse events during follow-up, including bleeding, paradoxical embolism, myocardial infarction, sudden death, and stroke. Results There were no significant differences in closure time, PCI time, and hospital stay between the TTE and X-ray groups (P>0.05). Post-treatment, cardiac function indices improved in both groups compared to pre-treatment (P<0.05), with no significant differences between groups (P>0.05). Shunt status also improved post-treatment (P<0.05), but differences between groups were not statistically significant (P>0.05). The complication and adverse event rates were similar between the two groups (P>0.05). The Log-rank test indicated no significant difference in clinical event-free survival between TTE-guided and X-ray-guided PFO closure combined with PCI (HR=0.783, 95% CI: 0.324-1.895, P=0.582). Conclusion For patients with PFO and coronary artery disease, TTE-guided PFO closure combined with PCI is as safe and feasible as X-ray-guided methods. TTE procedures can help reduce radiation exposure in clinical settings while maintaining good safety. [ABSTRACT FROM AUTHOR]
