Abstract

Cow's milk protein allergy (CMPA) is a reproducible immune reaction to exposure to cow's milk proteins. The diagnosis of CMPA is based on an oral food challenge (OFC) after an elimination diet, and other complementary tests such as skin prick test (SPT) and serum IgE measurement (sIgE) are described. This article provides a comprehensive and up-to-date overview of CMPA; its pathophysiology, treatment and diagnosis. With emphasis on diagnostic methods and clinical considerations to avoid underdiagnosis or supradiagnosis of this pathology. The importance of intestinal microbiota in CMPA is also emphasized, and some specific non-IgE-mediated syndromes such as PLV-induced proctitis or proctocolitis and PLV-induced enterocolitis (FPIES) are mentioned.
