Amphotericin-B: Lack of efficacy in histoplasma capsulatam : case report.

Abstract
- A 45-year-old man with a history of renal allograft recipient exhibited lack of efficacy following treatment with lipid emulsion of amphotericin B for pulmonary histoplasmosis. Initially, he was treated with unspecified antitubercular medication, but his symptoms persisted. He was later diagnosed with pulmonary histoplasmosis and treated with amphotericin B. However, after a week, he developed multiple episodes of gastrointestinal bleeding, indicating lack of efficacy. Despite undergoing conservative therapy and stabilizing initially, he was readmitted to the ICU with severe gastrointestinal bleeding, which worsened and ultimately led to his death due to postoperative sepsis with multiorgan dysfunction.
