Abstract

This document is a comment on an article about the use of AI and digital modeling for precision in positive end-expiratory pressure (PEEP) settings in intensive care medicine. The authors agree with the previous letter that the relationship between recruitability and PEEP setting is complex and requires further understanding. They suggest exploring options such as automated multi-parametric monitoring and modeling, as well as algorithms based on AI, to predict treatment response and minimize harm. However, they acknowledge challenges in determining input variables and understanding the systemic consequences of PEEP changes. They emphasize that technology can provide monitoring capabilities and potential solutions, but the goals of care should still be determined by clinicians in partnership with patients and their families.
