Abstract

Decision-making after a diagnosis of prostate cancer is based on many determining factors, one of the most important is undoubtedly the histological grade. The Gleason grading system for prostate carcinoma is the dominant method used worldwide in research and daily practice. To determine the relationship between tumor grade in needle biopsy samples and after a prostatectomy, the results of 33 patients who underwent both procedures in a hospital in the interior of Paraná were compared; using the Gleason classification modified by the International Society of Urological Pathology (ISUP). Differences were observed between biopsy and prostatectomy for primary Gleason pattern, secondary pattern and score. As evidenced by the overclassification variable and mainly by the subclassification of the pathological stage, showing the difficulties of using the Gleason pattern as the only prognostic and therapeutic decision factor. Gleason correlations were identical in 57.6% of cases, 33.4% differed by just one Gleason score grade, 6% differed by two or more score grades. The Gleason score of radical prostatectomy samples was higher than that of needle biopsy in 33.4% of cases and lower than that of biopsy in 9% of cases. [ABSTRACT FROM AUTHOR]
