Abstract

In 2023, the European Conference on Infections in Leukaemia elaborated guidelines for managing toxoplasmosis in hematology patients, mainly after allogeneic hematopoietic cell transplantation. Toxoplasma disease, although a rare event, has a high mortality, especially in case of cerebral involvement or disseminated form, and may be of very acute presentation. These guidelines highlight the need for a preemptive strategy based on a weekly screening by quantitative PCR from transplantation in seropositive recipients, allowing to early treat reactivation before the occurrence of toxoplasma disease. The purpose of this article is not to summarize these guidelines but rather to discuss critical issues which have been largely debated during the elaboration of these guidelines and which support the conclusions of the group.
