A 25-year-old pregnant woman experienced lack of efficacy while being treated with spironolactone for left-ventricular dysfunction. She had a history of abnormal electrocardiography findings and complained of palpitations and dyspnea during exercise. After switching to spironolactone, she developed decompensated heart failure and frequent nonsustained ventricular tachycardia.
