Spironolactone: Lack of efficacy and exposure during pregnancy.

Abstract

A 25-year-old pregnant woman experienced lack of efficacy while being treated with spironolactone for left-ventricular dysfunction. She had a history of abnormal electrocardiography findings and complained of palpitations and dyspnea during exercise. After switching to spironolactone, she developed decompensated heart failure and frequent nonsustained ventricular tachycardia. The woman underwent an emergency caesarean section and gave birth to a healthy baby. She later received an implantable cardioverter-defibrillator and was being considered for a heart transplant.
