基于“主动健康”理念的心脑血管疾病慢病管理模式.

At present, cardiovascular and cerebrovascular diseases are currently one of the chronic diseases with the heaviest disease burden in China. With the gradual aging of China's population, the traditional chronic disease management mode of "disease-centered" can no longer meet the health service needs of current residents. The chronic disease management model based on the concept of "active health" is a new type of "health-centered" chronic disease health management model. This mode has established a closer, more convenient and affordable communication mode between residents and family doctor teams.It saves the time, manpower and economic cost of family doctor team service, and is conducive to improving the quality and efficiency of chronic disease management and residents' health literacy. Taking cardiovascular and cerebrovascular diseases as an example, this paper discusseed the chronic disease management mode and research content based on the concept of "active health" aiming to explore the form and standard of "active health" chronic disease management mode, and provide reference for the promotion of this mode [ABSTRACT FROM AUTHOR]
