Supporting Patient and Family Engagement, Organization, and Health Literacy with the Health Care Notebook.

The article focuses on the Health Care Notebook, created by the librarians at Cook Children's Medical Center in partnership with Family Services, aimed at helping families navigate the stressful period after receiving a new chronic diagnosis or complex condition. Topics discussed include updated definition of health literacy according to Healthy People 2030 and five sections of the Health Care Notebook.
