Ocular Myasthenia: It's Worth a Second Look.

Abstract
- This article, published in the journal Neuropediatrics, discusses a case of ocular myasthenia, a common presentation of myasthenia gravis (MG) in children. The case presented challenges in diagnosis and treatment, requiring specialized laboratory tests and high doses of pyridostigmine. The patient initially had right-sided ptosis and diplopia, which worsened and showed rapid alteration. Despite negative ACh-receptor antibodies and normal CSF, MRI, and electrophysiology results, the patient was successfully treated with pyridostigmine. The article highlights the importance of live cell-based assays in confirming the diagnosis of ocular MG when other tests are negative, and suggests that higher doses of pyridostigmine may be necessary for successful treatment.
