Abstract

Morbidity and mortality associated with antimicrobial resistance is a global public health problem. Vancomycinresistant Enterococcus faecium, which is frequently seen in healthcare-associated infections, is a Gram-positive cocci that the he World Health Organization has given high priority on its list of priority pathogens with antimicrobial resistance. Recently, the rate of vancomycin-resistant Enterococcus faecium isolation in invasive infections has been reported as 15.8% in Turkey. The main mechanism of resistance to vancomycin is alteration of the D-alanyl-D-alanine terminal molecule in the target region, and decreased affinity for the antibiotic. The van gene responsible for vancomycin resistance is either chromosomally encoded or acquired by mobile genetic elements such as plasmid and transposon. The main mechanism of its spread is that mobile genetic elements can be transferred within the same species or between different bacteria like Staphylococcus aereus. In this review, data, extracted by PubMed and Web of Science Databases searching with the terms "Vancomycin-resistant Enterococcus faecium", "Vancomycin resistance mechanisms", "vancomycin resistance in Gram-positive cocci" is summarized and briefly discussed with recommendations.
