Periorbital cellulitis: report of a complication of odontogenic rhinosinusitis in an adult patient.

Abstract

Introduction: Odontogenic rhinosinusitis is commonly located in the maxillary sinus, a chronic disease whose initial treatment is antibiotic therapy and symptomatics. Complications are divided by affected site, with orbital complications being the most prevalent. Periorbital cellulitis restricts the preseptal location, routinely with oral antibiotics and observing the need for tooth removal. Objectives: Report the case of a patient who developed periorbital cellulitis, a complication of odontogenic rhinosinusitis. Resumed report: Female, 45 years old. Referred to otorhinolaryngology which found infection of odontogenic origin. Reports toothache for 14 days, progressing to worsening pain and swelling of the right hemiface, started treatment with Amoxicillin, 7 days without improvement. Hospitalized for 4 days using Clindamycin + Ceftriaxone. Physical examination shows large edema (3+/4+) hardened in palpation on the right hemiface with complete eyelid occlusion, hyperemia and redness. Increased volume extended to the submandibular, submental and temporal regions, also hardened to palpation and hyperemia. Associated with trismus and poor general condition of the teeth. Cranial CT showed maxillary and ethmoidal sinusitis. Dexamethasone was added, referral to oral and maxillofacial patients. Conclusion: Imaging exam confirms the diagnosis of maxillary and ethmoidal sinusitis, periorbital cellulitis as a complication. Corroborates studies that indicate it as the most prevalent cases of sinusitis in the ethmoid sinus due to the anatomical boundary between the ethmoid cells and the fact that the orbit is formed by a thin bone layer, the lamina papyracea, which facilitates the spread of the infection.
