面部基底细胞癌经外科手术联合艾拉光动力 治疗无效的影响因素分析.

Abstract
- Objective To analyze the influencing factors of ineffective treatment of facial basal cell carcinoma (BCC) through ella photodynamic therapy (ALA-PDT) combined with surgery. Methods Fifty patients with BCC underwent surgery combined with ALA-PDT treatment from January 2016 to December 2019 were selected. The ALA-PDT treatment was performed at the day of stitche sremoval at 1 week after surgery. The therapeutic effect was evaluatedat 6 weeks after ALA-PDT treatment, and all patients were divided into effective group and ineffective group. The baseline data between 2 groups were counted and compared, and the influencing factors of ineffective treatment were analyzed by logistic regression. Results After treatment, there were 45 patients in the effective group (90.0% effective) and 5 patients in the ineffective group (10.0% ineffective). Single logistic regression analysis showed that there was no significant difference in age, sex, duration of illness, lesion size, lesion location and number of lesions between the effective group and the ineffective group (P>0.05). The proportion of pathological type (nodular type), previous main working environment (outdoor) and Skin Grown Classification (Ⅱ aggressive activetype) in the ineffective group were higher than those in the effective group (P<0.05). Multiple logistic regression analysis showed that nodular type [OR=9.360, 95% CI (2.010,43.594)], previous main working environment (outdoor) [OR=8.167, 95% CI (2.388,27.927)], Ⅱ aggressive active type [OR=9.743, 95% CI (2.565,37.003)] were the influencing factors for the failure of surgery combined with ALA-PDT in patients with facial BCC. Conclusion There is still a certain risk of ineffective treatment of surgery combined with ALA-PDT for facial BCC. The nodular type, previous main working environment (outdoor) and Ⅱ aggressive active type may be the influencing factors of ineffective treatment.
