Abstract: The article offers information on the Transitions of Care: Surgical Care Navigator Program course. According to the article, the course was implemented in January 2018 and aims to provide students in the health professions with first-hand experience in interprofessional communication during the transitions of care of medically complex surgical patients. Student perceptions after course participation in the spring of 2020 is also mentioned.
