Abstract

Background Medical guidelines serve the purpose to transfer best available evidence from randomized controlled trials, meta-analyses and expert consensus into daily practice. They should facilitate decision making for attending physicians and patients to find the appropriate way of treatment in individual disease situations. This paper investigated the implementation of the ESC/ ESH guidelines for the management of arterial hypertension 2018 in family physicians' practices in Schleswig-Holstein. Methods In the summer of 2019 a cross-section of 1000 family physicians (specialists for family medicine, general practitioners, specialists for primary care internal medicine) from Schleswig-Holstein were postally interviewed by using a specially designed questionnaire, which was validated by this pilot survey. The data collection was organized in an anonymous form and analyzed by descriptive statistical methods. Results Data from 259 family physicians were evaluated. One year after their publication the new guideline recommendations on the management of arterial hypertension already have a high acceptance among family physicians in Schleswig-Holstein (> 68 % agreement). The practical implementation, however, achieved partially significantly lower rates. Conclusions Despite high acceptance among family physicians from Schleswig-Holstein the recommendations of the European hypertension guideline are only partially implemented. Especially an initial medical treatment with a two-drug combination as far as a general blood-pressure goal of 130-139/70-79 mmHg for all elderly patients over the age of 65 are considered critically. Implementation could possibly be improved by intensifying the discussion of evidence and implementation barriers as part of continuing medical education as well as by optimizing legal guidelines for medical prescription.
