SISTEMA DE MEDICAÇÃO HOSPITALAR: ANÁLISE DO CONCEITO.

Describe the concept of Hospital Medication System. Concept analysis about Hospital Medication System, according to the Walker and Avant method, through an integrative literature review of 48 scientific productions on the subject. The results suggest that there is divergence among researchers in the use of the concept and the use of several synonymous terms. Are presented in three thematic pillars: 1) definition attributes; 2) substitute terms and related concepts; 3) background and consequences. We can consider an operational definition of Hospital Medication System as a complex, organized and multi-professional system with interdependent patient-focused processes that share the common goal of safely, effectively, appropriately and efficiently providing medicines to patients, involving doctors, pharmacists and nurses, patients and relatives/caregivers, with multiple stages and multiple passages of responsibility, influenced by the organizational and care environment.
