Abstract

The case of a 43-year-old adult patient with a severe skeletal class III malocclusion (Wits -9.1mm), anterior crossbite and skeletal compression of the maxilla of 6mm by WALA is presented. The performed treatment in this patient consists of a skeletal maxilla disjunction without SARPE with the aid of a pure bone-borne supported expander (MICRO4-expander™) following a specific and personalized very slow disjunction protocol for adult patients. We call this the activation protocol with polycyclic force control (CFPC protocol, explained in a 2 PART). Once the disjunction of the maxilla has been completed and its desired width is achieved, the case is finalized with an orthognathic surgery that consists of an advancement of the maxilla followed by a reduction mentoplasty. In the edentulous areas, conventional dental implants were placed that served as anchors to solve the lower dental crowding with a discrete distalization of the lower premolars.
