Abstract
- Objective: To explore the clinical application value and advantages of micro-neurosurgical treatment of cerebellopontine meningioma under "microendoscope combined". Methods The clinical data of 39 patients with cerebellopontine angle meningioma treated by the retrosigmoid sinus approach through the suboccipital were retrospectively analyzed. Among them, 11 cases used "micro-endoscopy combined" surgery (microendoscopy combined group), and 28 cases used simple microscope surgery (microscope group). Results In the combined microendoscopy group, 9 cases of simpson Ⅰ and Ⅱ resection were performed, and 2 cases were subtotal resection. The total tumor resection rate (Simpson grade I and II) reached 81.8%. In the microscope group, 19 cases of simpson Ⅰ and Ⅱ were resected, 5 cases were subtotal resection, 4 cases were partial resection. The total tumor resection rate (Simpson Class I and II) reached 67.9%. Both groups achieved facial nerve anatomy preservation. For meningiomas that involve the inner ear canal (IAC) and are located in front of or behind the inner ear canal, the retention rate of facial nerve function in the microendoscope combined group is 71.4%, and the retention rate of facial nerve function in the microscope group is 66.7%. Conclusion The "microendoscope combined" surgical method is an effective method for the treatment of cerebellopontine meningioma, especially for cerebellopontine meningioma that grows to IAC or extends to the tentorium. The combination of neuroendoscopy and microscope can complement each other, increase the total tumor resection rate, and reduce nerve function damage. [ABSTRACT FROM AUTHOR]
