Abstract

BACKGROUND: The spectrum of skin lesions in a patient with rheumatoid arthritis is varied, they can be specific and non-specific. Reactive perforating collagenosis is characterized by transepidermal elimination of degenerated collagen fibers, its association with rheumatoid arthritis is rare. CLINICAL CASE: A 59-year-old female patient with a history of rheumatoid arthritis who came to the clinic for 1-year evolution dermatosis in the extensor region of the forearms, back of hands, anterior aspect of thighs, legs and back of feet, characterized by hyperpigmented papules with a keratotic plug. An excisional biopsy was performed of one of the lesions, a crater-shaped lesion with epidermal hyperplasia was observed at the edges; the center consisting of a keratin plug, cellular debris and absence of epidermis, below which the papillary dermis exhibited collagen fibers with degenerative changes oriented perpendicularly to the epidermal defect, as well as a dense infiltrate of polymorphonuclear cells and lymphocytes. Topical steroid treatment was started showing improvement. CONCLUSIONS: Acquired reactive perforating collagenosis is a rare condition and much less frequent in association with rheumatoid arthritis.
